Required Physical Exams

A physical exam is required by Nebraska law for all students entering school for the first time (early childhood or kindergarten), entering seventh grade and all students transferring into Omaha Public Schools from out-of-state or country. Required school physical exams must be completed March 1 or later to be valid for the upcoming school year.

Students who wish to participate in athletics will also be required to complete a sports physical. Below are the guidelines for each grade level where a physical is needed. 

Early Childhood/Kindergarten or Out-of-State/Country Transfers:
A physical exam and vision exam is required for all students entering school for the first time (early childhood or kindergarten) or transferring from out-of-state or country. The vision exam may be completed by a health care provider or an optometrist. A dental exam is also recommended. Your child's health care provider will give you the documentation required for a school physical. 

Seventh Grade:
A physical exam is required for all students entering seventh grade. No vision exam is required. Completing the seventh grade physical exam also meets the requirement for athletic participation in after-school sports. Your child's health care provider will give you the documentation required for a school physical. 

Athletic Participation (Eighth-12th Grades)
Students who wish to participate in after-school sports for eighth through 12th grade are required to have a sports physical. To meet requirements, the physical must be dated May 1 or later. If your child is participating in competitive athletics, a physical examination by a licensed health provider is required and signing a waiver does not satisfy this requirement. Exemption Waiver
If you do not wish to provide evidence of a physical or vision exam, a signed statement waiving this requirement must be submitted to the school. Contact your school nurse for more information.
